To download forms and applications for the OHIP, visit www.health.gov.on.ca/english/public/forms/form_menus/ohip_fm.html The application for a new health card must be accompanied by three original documents to prove the expatriates residency in Toronto and identity. The application has to be submitted by all above the age of 15 years.
